Student and Parent/Legal Guardian will need to make an appointment with the DMV in order to obtain their permit.  The cost is $21.50 ($15.00 + $6.50 administrative fee). Applicants must pass the required vision test and provide the following: 

1. Certified Original Birth Certificates (available from the Bureau of Vital Statistics) 

2. Acceptable photo identification (this can be a school ID card)

3. Social Security Card or Number for verification 

4. Liability Signer (if under 18) – must be birth parent or court-ordered guardian 

5. Verification of School Compliance (VOC) (if under 18) (You can call the office of the school and ask for one)
